Please fill in any fields that are applicable to your data and leave the rest blank. You must, however, fill in the fields prefaced by an asterisk *.

*Standardized Name (Surname, Forename) (required)

Document Name (spelling as it appears in the source document)

Title (title or status noted in the document source, such as Sir, Lady, Master, esquire)

Identifier (A=apprentice; C=clergy; G=gentleman/gentry/nobility; J=Jewish; M=craft master; O=orphan; W=woman; V=widow or widower; X=alien)

Craft (guild/company)

Cft_Flag (A code that indicates the credibility of the information in the Craft field based on this source record; see the Database Guide for options)

Craft Office (if specifically referenced in the source document)

Occupation

Occ_Flag (A code that indicates the credibility of the information in the occupation field based on this source record, see the Database Guide for options)
